Our Promise • To raise awareness of colorectal cancer (CRC), the importance of early detection and the most effective screening methods available; • To increase awareness of CRC risk factors and factors that may help prevent CRC, including knowing one’s family/genetic cancer history, exercise and nutrition; • To reduce the disparity that exists in CRC incidence and mortality rates by providing funds to improve screening rates in underserved and uninsured communities; • To award translational research grants to young investigators studying genetic and epigenetic changes that will ultimately lead to new breakthroughs in the prevention of, therapy for and elimination of CRC in our lifetime; • To leverage the impact of our donors’ support by developing broad public/private partnerships with local, regional, national and global organizations invested in a mutual goal of colorectal cancer prevention and early detection.

One’s basic tasks of daily life accomplished – from bathing to bill-paying. Social supports that prevent isolation. Our mission and values have remained steadfast since 1971. We believe that older adults are valuable contributing members of their communities, who are entitled to live as independently as possible with dignity and personal choice. We believe that professionally delivered home-based and community-based social services are essential for maintaining the health and wellbeing of older adults. We believe that support for family caregivers is critical in keeping older adults healthy, at home, and connected to family and community. We believe that engaging community volunteers to provide social interaction for their older neighbors contributes to increased health, satisfaction and longevity. We believe that by bringing youth and older adults together in intergenerational programs not only gives youth a knowledge and appreciation of the challenges of aging, but also strengthens the bonds between generations and brightens the lives of older Brooklynites.

SUNNYVALE COMMUNITY SERVICES

Sunnyvale Community Services (SCS) is designated as the Emergency Assistance Network agency (EAN) for all zip codes in Sunnyvale, CA. As the safety net agency in our community, SCS addresses immediate critical needs to help families and seniors stay housed with food on the table and utilities turned on as they work toward self-sufficiency. We serve every eligible client who comes to us for help, never turning anyone away due to lack of resources - even though that has become increasingly challenging with the rapidly growing need for our help. Our clients are Sunnyvale residents and homeless families. Last year we served over 6,800 individuals, disbursing nearly $800,000 in financial aid. Families receiving financial aid received a personalized 3-month budget to help them get back on track. As one of the largest partners of Second Harvest Food Bank, we engage over 1,000 volunteers help to distribute 850 tons of nutritious food and fresh produce annually. In the last three years, SCS has built a strong foundation in comprehensive case management for homeless individuals as well as seniors and families in crisis. We have collaborated on new health and wellness programs to support healthy nutrition and financial education and credit repair to help clients become self-sufficient. In 2015, we launched our Supportive Services for Veterans Families, funded by the U.S. Veterans Administration to assist veterans who are homeless or at risk of homelessness to move into stable housing.
